Personal Information Protection Basic Act

― Basic policy on personal information ―
 

The hospital handles and protects personal information accurately and safely.
Considering this as our social responsibility, we hereby declare the following:
 

 

1. Regarding the collection, use and provision of personal information, our hospital respects the rights of individuals regarding personal information, specifies the purpose of collection and limits of use, and strives for appropriate management.
    

2. The hospital will establish and operate a compliance program for the protection of personal information, and all representatives and staff will comply with it to ensure proper management of personal information.
    

3. The hospital will implement safety measures to prevent unauthorized access to personal information, loss, destruction, falsification and leakage of personal information.
    

4. In addition to conducting education and awareness-raising activities within the hospital regarding the importance of personal information protection, our hospital will establish a personal information protection manager/auditor, establish an appropriate management/audit system, and continue to operate it.
    

5. Our hospital complies with Japanese laws and regulations and other norms regarding personal information.
    

6. The hospital will continuously review and improve the content of the above activities.
